Requirements for TOURIST VISA
•    Completely filled up application
•    Valid passport 6 months from the date of travel includes old passport
•    Two (2) recent colored 4.5 cm x 4.5 cm size photos with white background - Applicant is suggested to be in

     decent attire, preferably dark colors with collar and sleeves
•    Day to day schedule in Japan
•    Original NSO birth certificate issued within the year from the time of filing (without previous and used Japan

      visa)
•    Original NSO marriage certificate issued within the year from the time of filing (without previous and used Japan

     visa)
•    Certificate of employment stating position, tenure of service and gross annual income withapproved and signed

      leave of absence (FOR EMPLOYED); DTI and/or SEC Business Registration Certificate (FOR BUSINESS OWNER)
•    Original bank certificate
•    Copy of Income Tax Return
•    Flight Itinerary
•    A letter from the applicant authorizing La Casa Viajes Travel and Tours. to file and claim your visa in your behalf. 

Remarks
•    For NSO Birth Certificate under “late registration” (born from 1940 to present),additional original documents

      required:
•     Baptismal Certificate
•    Form 137
•    For NSO birth certificate with “unreadable details” and or if “no record found”, additional original documents

      required:
•    Certified True Copy of the Birth Certificate from the Local Civil Registrar

Please do not leave any blank in the application form. Kindly put "N.A." if it's not applicable.           
Make sure that the back page of the passport is completely filled-up (address, tel numbers and  name of person to be contacted in case of emergency.
