•    Duly filled up online Application form
•    Passport valid for the next 6 months
•    2X2 pictures (2 pcs)
•    Bank Certificate- must provide proof that you have enough money for your visit to US
•    Income tax return(ITR)
•    Land titles, car registration, stocks etc.
•    Employment certificate and Application for leave of absence / business registration
•    Photocopy of credit cards / statement of account
•    Proposed itinerary
•    Invitation letter ( from your sponsor/relative/friend – if applicable )
•    Marriage Certificate / Birth Certificate (NSO authenticated)
•    VISA FEE-  subject to change depending on the exchange rate (USD 150)
•    Call BDO to verify exact amount in peso
